If a HealthForce Physician needs information from your personal healthcare provider, you will receive a Provider Letter request by e-mail.
WHAT IS A PROVIDER LETTER?
The Provider Letter is addressed to your personal healthcare provider explaining what information the HealthForce Physician needs to help move your case forward.
WHAT YOU NEED TO DO
1 Log into www.MyHealthForce.com.
2 Click the Documents link in your case.
3 View and download the Provider Letter.
4 Send the letter to your personal healthcare provider, requesting that they respond to HealthForce ASAP.
5 Your personal healthcare provider must respond directly to HealthForce.
6 You will receive a notification when your case is closed. Log into www.MyHealthForce.com to confirm that your
healthcare provider responded to HealthForce. Click the Documents link on your Home Page.

The letter cannot be e-mailed to you because it is not a secure way to transmit your private health information.
Contact HealthForce if you would like us to securely fax the Provider Letter to your doctor for you.

MULTIPLE LETTERS
Depending on your health history, you may be issued more than one Provider Letter.
